Vehicles Tool Kit for OSU Vehicles

In the Event of an Accident


...the OSU driver should do the following:

At the scene of the accident, complete the "At the Scene of the Accident Form". Have any witnesses complete the "Witness Cards".

Immediately (or as soon as possible) contact OSU Office of Risk Management (ORM),
Risk Officer at (541) 737-7252 or email Risk  Complete the DMV "Traffic Accident and Insurance Report". Use the information on the "At the Scene of the Accident Form" to complete this form. It is the driver's responsibility to submit the original of this report to the State Vehicle Division DMV within 72 hours for accidents that involve any injuries and/or damages that exceed $1,500.

(For accidents occurring out of Oregon, follow the same procedure except contact that State's DMV or DOT to obtain their State Traffic Accident and Insurance Report Form).

  • One for OSU Motor Pool (if it is a Motor Pool vehicle), or the department supervisor for any other vehicle.
  • One for yourself.
  • Mail original to Oregon DMV (address on form).

Make two copies of the DMV report for:

Complete the State Self Insurance Claim Form and have the department supervisor sign the form. Submit this form along with the necessary claims documents such as photos of OSU vehicle, police report, etc. (for a complete list of these claims documents refer to this page) to Motor Pool if the vehicle is a Motor Pool vehicle. If the vehicle is a department owned vehicle submit to OSU Office of Risk Management (ORM) at 644 SW 13th St.; Corvallis, OR 97333 or email risk@oregonstate.eduwith documents attached. Don't delay!

Obtain new accident report packets from the OSU Motor Pool, the department Supervisor, or below and replace in vehicle.

Proof of Insurance


All University vehicles should have a proof of insurance certificate in the vehicle.

At the Scene of Accident Information Form


Information forms on process for accident procedures and obtaining information at the scene of the accident should also be in the vehicle. Please restock this form upon use.

Witness Cards


Under Oregon Revised Statute 811.715, a witness of a traffic accident is obligated to provide their contact information. Please place these witness cards in the vehicle and restock upon use.

DMV Reporting Form


The Department of Motor Vehicles requires that all accidents greater than $1500 or that involve an injury be reported to the State Vehicle Division within 72 hours. This form is found at: http://www.odot.state.or.us/forms/dmv/32.pdf

OSU Risk Management Reporting Form


To report claims involving loss and/or damages to University owned vehicles, complete the State Self Insurance Claim Form and report vehicle losses to OSU Risk Management the following business day.
