Filing a Claim
To report claims involving property loss and/or damages to University owned property, click here for the State Self Insurance Claim Report Form. All property losses, including state vehicle losses and losses under our deductible limit, must be submitted on this form and can be saved and attached to an email and submitted to: risk@oregonstate.edu

If the loss is due to a theft, break in, or vandalism, these incidents are criminal and should be reported to law enforcement immediately upon discovery. If it occurred on the University Campus, contact the Department of Public Safety at (541) 737-3010. If it occurred elsewhere, contact the police department for that jurisdiction. The Self Insurance Claim Report Form will need to be completed by the department and returned to OSU Office of Risk Management (ORM). It is not the responsibility of the Department of Public Safety to file the Self Insurance Claim Report Form.
OSU's property coverage deductible is $2500. Please see Reporting an Incident below for reporting requirements.
The below information will be needed; however do NOT wait until you have this information to file a claim!
- A copy of any police report
- A copy of any summons and complaint
- Pictures of any damage to property (all sides of property, including damages)
- A detailed list of damaged or lost property
- Copies of email messages or letters regarding the incident
- Documentation of replacement values for lost/damaged equipment in the form of invoices, receipts, etc.
- Documentation of two estimates showing replacement values for lost/damaged equipment (example: screen-prints from websites, or copies of pages from catalogs)
- Witnesses names or other material you may have related to the claim
Reporting an Incident
OSU departments are responsible for reporting all incidents. Often several departments may have experienced the same type of loss (i.e. theft and vandalism); however, it is unknown to each other. If there are several losses due to one cause, this can be reported as one claim; therefore only one deductible of $2500 is applied and prorated between departments. Reporting all incidents will allow OSU Office of Risk Management (ORM) to report these types of losses as one. Also, some claims qualify for restitution that can be sought from a third party or subrogation rights with the other parties involved.
